The injured employee was talking to the other driver when the second forklift drove forward and pinned the injured employee's foot between the two forklifts. An employee was installing an ice machine at a customer's location. While doing so, he bumped his left elbow on the machine and received a small scratch. The next day it started to swell and turn red. He was hospitalized for an infection in the wound.

An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

An employee was performing duties as an expeditor. After opening the dock door, the employee scanned the barcode on the door of the truck and placed one foot on the truck and one foot on the dock. The truck drove out of the stall, causing the employee to fall 4 feet off the dock onto the concrete. The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

An employee was operating a stand-up reach truck. He was taking a small pallet of product through a dock area when another stand-up reach truck struck him. The steel forks punctured his lower right calf, also breaking a bone in the same area. He was hospitalized, requiring surgery.

On October 8, 2024, an employee was wiping down the core cutter machine when his cleaning rag was caught by the running belt pulling his right ring finger into the belt assembly and resulting in a partial amputation.
