An employee was hooking up the lines from a trailer to a yard truck when she lost her footing and fell onto her right side on the ground. The employee was hospitalized with an injury to her right hip.

At about 3:40 a.m. on August 13, 2025, an employee was filling orders in an aisle. She was pulling over her material handling equipment to allow another employee to get around her when the equipment struck a rack. She suffered a laceration to the left leg/knee area.

An employee was operating a rolling container carrier (RTC) vehicle in pedestrian mode. When he operated the hand lever to turn it, the RTC pinned his left leg against the pick location. The employee suffered a laceration on his left knee and was hospitalized.

On October 9, 2024, an employee was observing a weekly store delivery. While the liftgate was on the ground, a rolling container carrying refrigerated product rolled off. When the employee stopped the rolling container, it fell onto her left leg. She was hospitalized with a left leg fracture.

An employee was unloading freight from a semi-truck trailer when their foot was caught on the liftgate, resulting in a partial amputation of their left big toe.

As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

A temporary employee was punched in the face by another temporary employee. He suffered a laceration to the eye.

An employee was backing up a powered industrial vehicle. Her left hand was caught between the vehicle and a rack, and the wrist was broken.

After rewiring the new spool, the employee's clothing got caught in the turning shaft when the left spindle started back up. The employee's left arm was pulled into the machine, resulting in a fractured humerus and lacerations to her triceps. She was hospitalized and required surgery.

An employee was resetting a warehouse racking system utilizing an order picker. They fell approximately 5 feet from the order picker to the concrete floor and sustained fractured ribs.
