An employee suffered a fingertip amputation to the left middle finger while performing maintenance on a Bobcat.

At about 1:15 p.m. on October 12, 2022, an employee was climbing a fixed ladder to disembark from a barge at a dock. He slipped and fell from the ladder to the deck of the barge about 5 to 7 feet below. His left leg was broken and he was hospitalized.

A sample probe became plugged with corn. Two employees began unclogging the corn in the probe. One of them went outside to the probe, then directed the other to open and close the probe, releasing the clogged corn. When most of the clogged debris had fallen out, she inserted her right index finger into one of the openings on the probe. The probe was turned on again and shut on her right index finger, causing a partial amputation.

An employee was changing a saw blade when the saw machine activated. The employee's left ring and little fingers were amputated.

An employee was clearing a jam from the label cutting machine when their left middle finger was partially amputated.

Two employees were repairing the hydraulic engagement pins on a rented front-end wheel loader. The machine controls were activated to move the attachment pins and an employee's finger was caught between the pins and the bushings. Their right index finger was partially amputated.

An employee was cleaning the conveyor on a piece of equipment when they slipped and their right hand was pulled into the chain sprocket. The employee's fifth fingertip was amputated.

Alsco Inc.

An employee was throwing blankets onto a blanket folding and stacking machine. A blanket got stuck in the machine and she attempted to remove it when the machine contacted her right little finger, resulting in a fingertip amputation.

Faler Feed Store, Inc.

An employee was preparing to unjam an auger shaft while standing on a forklift-elevated, job-made platform. The employee's wrench slipped off the equipment, causing them to lose their balance and fall. The employee landed on the ground 14 feet below and suffered fractures to the orbital bone, left elbow, and lower back.

An employee was walking past a crossover conveyor when a ramp came down and hit them in the back. The employee sustained a broken back vertebra, as well as a concussion, and was hospitalized.

Alliance Ag and Grain LLC

An employee bent over to pick up a rubber mat from the ground. When he grabbed the corner of the mat, a rattlesnake hidden under it bit his left middle finger near the first knuckle. He was hospitalized.

An employee had been cleaning a grain bin. He walked across the bin and his left foot contacted an unguarded floor auger, resulting in severe lacerations to the foot. The auger was not locked out/tagged out at the time.

On September 23, 2025, an employee was monitoring a feed roller when they tripped and their right hand contacted the roller. The employee was hospitalized and their right hand was amputated at the wrist.

Takeda Pharmaceuticals U.S.A., Inc.

An employee fell while moving a water canister, resulting in fractures to four fingers on her right hand.

Great Dane Ltd. Partnership

An employee was changing a die in a press when the die slipped and crushed the employee's left index finger. The employee sustained an open facture of the tuft of the left distal phalanx and a partial amputation.

An employee was mixing sauce using a sauce barrel mixer when his left little finger was amputated.

AbbVie Inc.

An employee was driving a boom lift (in the lowered position) in an exterior dock area. The lift s left wheels rolled onto base plates that covered a 3-foot-deep sump pit. The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.

A temporary employee was carrying a nail gun through the warehouse when it hit his leg, activated, and fired a nail into the leg.
