Final narrative
On September 2, 2015, at approximately 3:15 p.m., a maintenance employee was attempting to adjust a moving conveyor that was off track (not centered) when he tripped on dry clay that had fallen from the conveyor onto the floor and fell into a slow moving conveyor, fracturing his right arm and requiring hospitalization.
An employee arrived at work and was walking into the store through the parking lot. The employee tripped over a curb and fell to the ground, sustaining a femur fracture.
An employee was carrying a bucket of water to the work bench when he tripped. His right foot became stuck between the base of the 28" mixer re-screener (machine was not in operation) and anchor bolts protruding from the floor. The employee fell with his foot stuck, resulting in a fractured ankle that required surgery and hospitalization.
An employee was on a step ladder, loosing a bolt connected to a machine. He fell off the ladder, landed on his back less than 4 feet below, and hit his head. The employee sustained head trauma and was hospitalized.
