An employee was cleaning an inclined conveyor that was approximately 9 to 10 feet off the ground. The employee used a 10-foot A-frame fiberglass step ladder to access the working area. The employee's left hand got caught in the conveyor, resulting in a left wrist fracture. LO/TO was not performed at the time of the incident.
An employee was pushing a jammed pizza case down a conveyor system. The employee's left hand was caught between the case and the end stop of the conveyor; the employee suffered fractures and lacerations to the little, ring, and middle fingers.
