On October 28, 2024, an employee noticed a large carrot in a cutter bowl was not feeding into the slots on a carrot processing line. He went to remove the large carrot and his left middle finger contacted the cutter blades, resulting in amputation of the fingertip pad.

An employee was walking on a crossover at a pallet conveyor. They slipped and their foot became stuck between the rollers. The employee suffered a laceration that required surgery.

An employee was pushing 1,300-pound totes of product down a conveyor when his foot was caught between the conveyor and the roller. The employee sustained a foot injury and was hospitalized.

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.
