An employee was preparing a tank car for loading while chipping away solidified product in the manway opening of the tank car. A pressure noise within the tank car startled the employee, causing him to stumble backward and fall approximately 10 feet over the guard railing at the chemical loading rack to the ground. The employee suffered fractured vertebrae, a fractured right wrist and ankle, and a dislocated jaw.

An employee was installing an electrical tray remover from a ladder. As he was descending, his missed a step and fell 6-7 feet to the floor. The employee suffered a head injury.

Two employees were installing spray foam insulation in the attic of a new residential construction. The injured employee fell 8 feet from an A-frame ladder to the floor. The employee suffered lacerations, abrasions, and a head injury.

An employee was on a raised platform attaching wires to electrical boxes. The employee fell approximately 8 feet to the floor below and struck an exit sign on the way down, resulting in an injury to his face.

An employee was in a forklift man basket fixing a garage door opener when they fell approximately 8 feet to the concrete ground below. The employee suffered head and back injuries as well as kidney bruising.

An employee had just taken a zero-turn riding mower off a truck and parked it on the concrete ground. As he dismounted, his foot became caught on the equipment and he fell. He braced himself with his right arm, resulting in a fracture, a dislocation and a lateral collateral ligament (LCL) tear. He was hospitalized, requiring surgery.

An employee was adjusting the low/high speed screws on the carburetor of a push mower when the mower shifted backward and the employee's right-hand fingertips came in contact with the moving mower blades. The employee sustained a laceration to a fingertip and two fingertip amputations.

On April 25, 2024, around 10:00 a.m., a service technician was pulling on a rope to relieve the belt tension on a mower deck to remove the idler spring when the rope broke and he fell backward, fracturing his right leg/ankle.

On 1/20/2021, an employee was unloading a box truck with a pallet jack when he slipped on ice and struck his knee on the pallet jack. The employee suffered muscle detachment from his kneecap that requires surgery.

An employee had been loading a safe with a forklift. The employee was adjusting the forks of the forklift when their finger got caught between the forks. The employee sustained an amputation to the left ring finger.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
