An employee was walking back to the production area after leaving the break area when his foot or the edge of his pants got caught onto a pallet causing the employee to trip and fall to the concrete floor. The employee broke a hip as a result of the fall.

An employee was changing a water hose on a die machine. The employee had extended the ram of the machine fully to gain access to the water line. As the employee was checking the water hose, the hydraulics on the ram released, causing the ram to retract to its holding position. The ram caught the employee's hands, pinned them, and broke bones in both hands.

An employee was on a fiberglass step ladder, about 3 feet off the ground, capping off a dead air line. The employee fell off the ladder and suffered two broken ribs, a punctured lung, and head trauma.

An employee was asked to help perform maintenance on a die cast machine. The employee entered the work area, got into a kneeling position, and began using an open-end wrench to remove a hydraulic hose so a fitting could be repaired. When the hose came free from the fixture, residual hydraulic pressure released and hydraulic fluid struck the employee in the chest. The employee fell backward and struck his head on surrounding machinery. The employee was hospitalized with a subdural hematoma, fractures to the forehead, orbital area, and ribs, and required stitches for a forehead laceration.
