An employee was working on a belt-driven coolant pump when his left hand was caught in the belt, resulting in lacerations and fractures. He was hospitalized and required surgery.

An employee was on a fiberglass step ladder, about 3 feet off the ground, capping off a dead air line. The employee fell off the ladder and suffered two broken ribs, a punctured lung, and head trauma.

An employee was asked to help perform maintenance on a die cast machine. The employee entered the work area, got into a kneeling position, and began using an open-end wrench to remove a hydraulic hose so a fitting could be repaired. When the hose came free from the fixture, residual hydraulic pressure released and hydraulic fluid struck the employee in the chest. The employee fell backward and struck his head on surrounding machinery. The employee was hospitalized with a subdural hematoma, fractures to the forehead, orbital area, and ribs, and required stitches for a forehead laceration.

A trim die was being transported by a forklift. When the die was placed on the ground, the top part of it popped off and crushed an employee's hand against a set of metal stairs. The employee suffered a laceration to the right hand between the thumb and the index finger.

An employee was checking for a leak in a hydraulic hose on the trim press machine and lacerated his right middle finger.

An employee was changing a saw blade when the saw machine activated. The employee's left ring and little fingers were amputated.

An employee was clearing a jam from the label cutting machine when their left middle finger was partially amputated.

Two employees were repairing the hydraulic engagement pins on a rented front-end wheel loader. The machine controls were activated to move the attachment pins and an employee's finger was caught between the pins and the bushings. Their right index finger was partially amputated.

An employee was cleaning the conveyor on a piece of equipment when they slipped and their right hand was pulled into the chain sprocket. The employee's fifth fingertip was amputated.

An employee was throwing blankets onto a blanket folding and stacking machine. A blanket got stuck in the machine and she attempted to remove it when the machine contacted her right little finger, resulting in a fingertip amputation.

An employee was moving a hoist to the end of a bridge rail to create space for another hoist on the same rail. The hoist being moved, along with its supporting beam, detached from the bridge rail and fell, striking the employee in the head. He suffered a contusion and laceration to the head and was hospitalized.

An employee was operating a machine. When they performed a motion to push a machined engine block away from their body, they experienced a back strain, resulting in hospitalization.

On September 13, 2025, an employee had been working at an outdoor event. At the end the day, she was walking around outside the facility to make sure everything was put away and felt overheated. The employee was hospitalized for heat-related illness.

An employee was installing insulation in a wall panel when the panel fell on top of them, resulting in leg and back injuries that required hospitalization.

An employee put a cylinder machining pallet into manual mode to unjam it. After completing the task, their left hand became lodged between a pulley and belt, resulting in the amputation of three fingers.

An employee slipped on condensation on a walkway in a parking garage. She fell and suffered a closed fracture to the neck of the left femur.
