An employee was unloading a customer's dump truck with grain in it when the pneumatic doors on the back of the truck opened, catching the employee's finger between the chain and the door. The distal portion of his left index fingertip was amputated.

A truck driver fell from his truck and sustained fractured ribs, thumb, back, and face, along with a brain bleed.

An employee was cleaning a crumble maker roll machine when his left hand entered the machine and was drawn into the roller, amputating four fingers up to the last knuckle. The machine was running at the time of the incident.

An employee was driving a fertilizer tender truck and moving over to accommodate traffic when the truck rolled over. The employee suffered lower arm avulsions.

Employees were using a fork truck to move a fertilizer hopper when the hopper fell on one of the employees, resulting in a crushed pelvis and ribs.

An employee was preparing a tank car for loading while chipping away solidified product in the manway opening of the tank car. A pressure noise within the tank car startled the employee, causing him to stumble backward and fall approximately 10 feet over the guard railing at the chemical loading rack to the ground. The employee suffered fractured vertebrae, a fractured right wrist and ankle, and a dislocated jaw.

An employee was installing an electrical tray remover from a ladder. As he was descending, his missed a step and fell 6-7 feet to the floor. The employee suffered a head injury.

Two employees were installing spray foam insulation in the attic of a new residential construction. The injured employee fell 8 feet from an A-frame ladder to the floor. The employee suffered lacerations, abrasions, and a head injury.

An employee was on a raised platform attaching wires to electrical boxes. The employee fell approximately 8 feet to the floor below and struck an exit sign on the way down, resulting in an injury to his face.

An employee was in a forklift man basket fixing a garage door opener when they fell approximately 8 feet to the concrete ground below. The employee suffered head and back injuries as well as kidney bruising.

An employee was preparing to unjam an auger shaft while standing on a forklift-elevated, job-made platform. The employee's wrench slipped off the equipment, causing them to lose their balance and fall. The employee landed on the ground 14 feet below and suffered fractures to the orbital bone, left elbow, and lower back.

An employee was walking past a crossover conveyor when a ramp came down and hit them in the back. The employee sustained a broken back vertebra, as well as a concussion, and was hospitalized.

An employee bent over to pick up a rubber mat from the ground. When he grabbed the corner of the mat, a rattlesnake hidden under it bit his left middle finger near the first knuckle. He was hospitalized.

An employee had been cleaning a grain bin. He walked across the bin and his left foot contacted an unguarded floor auger, resulting in severe lacerations to the foot. The auger was not locked out/tagged out at the time.

On September 23, 2025, an employee was monitoring a feed roller when they tripped and their right hand contacted the roller. The employee was hospitalized and their right hand was amputated at the wrist.

An employee was readjusting a safety hook to remove hardware from gang forms when his feet slipped. He fell to the ground, resulting in a broken left arm and wrist.

An employee was on a step ladder in a store aisle, making room on shelving for merchandise. She fell from the step ladder to the floor, suffering two fractures in her left leg. She was hospitalized, requiring surgery.

An employee was closing the side cargo door to a company van. The tip of his finger got caught between the door and the door jamb, resulting in a partial amputation.

An employee lost his footing while climbing an inclined ladder. He stumbled down one step and was able to stabilize himself by holding the railing and twisting his body, but he suffered an internal abdominal injury and was hospitalized.

An employee was manipulating dies on a brake press. The press caught and crushed the employee's right forearm, resulting in hospitalization.
