Final narrative
A mold operator was hospitalized when he fractured his pelvis during a fall. The injured employee along with another employee was emptying a bin (30 gallon waste container) into a Gaylord box (1 yard cardboard box), which rests on top of a 6 inch riser. It is believed that the employee's toe became trapped underneath the riser and when the employee turned around after placing the bin on the riser he fell and landed on his hip causing the left side pelvic fracture.
A maintenance employee was working on a overhead crane from a ladder, installing a part. The crane moved unexpectedly and the employee fell from the ladder fracturing his left leg.
An employee was pulling on a bale of corrugated cardboard boxes that were on a conveyor. The straps on the bale failed and the employee fell to the floor. The employee was hospitalized and required surgery for a fractured femur and/or hip.
On December 27, 2023, an employee was delivering mail to an apartment complex when their knee gave out and they fell to the ground. The employee was hospitalized with a left tibia fracture.

The injured employee was letting air out of a winding shaft using scissors as a lift table was used to pull the roll off the shaft. The roll/cardboard core slid over the top of the injured employee's scissors and caused the scissors to amputate the employee's little fingertip at the nail base.
An employee was dropping off a skid of laminate to be cut at a table saw. The skid was lifted and the forks tilted forward when the stack slid forward and struck the injured employee. The employee was pinned between the laminate and the table of the saw, resulting in a back fracture and a laceration to the liver.
An employee was using a machine to cut a roll of plastic sheeting. The employee's right hand was at the end of the roll, and when the cutting mechanism of the machine cycled, the employee's right middle and ring fingers were cut, resulting in amputations to both fingers.
An employee was running acrylic film through an automated winding machine which wraps film around a cardboard cylinder core. He noticed that the film had slipped off the winder. He went to the front of the machine to re-attach the film to the winder spool. His fingers got caught between the film and the spool and his left arm was pulled up by the winder spool. The employee was hospitalized with a broken left forearm. Another employee injured their shoulder when pulling the first injured employee out of the machine.
An employee was readjusting a safety hook to remove hardware from gang forms when his feet slipped. He fell to the ground, resulting in a broken left arm and wrist.
An employee was on a step ladder in a store aisle, making room on shelving for merchandise. She fell from the step ladder to the floor, suffering two fractures in her left leg. She was hospitalized, requiring surgery.
An employee was closing the side cargo door to a company van. The tip of his finger got caught between the door and the door jamb, resulting in a partial amputation.
