Final narrative
A temporary employee had his left hand middle finger amputated while using a cut-off saw to cut up scrap OSB on or about October 17, 2015.
An employee was processing material on a roll mill. The machine jammed, then started to make a cut as the employee was trying to clear the jam. Its die crushed the employee's arm.
An employee was putting together two pieces of steel using a sheet metal joining machine. His right thumb slipped and was pinched as he lowered the die set. The thumb tip was amputated.
An employee was troubleshooting a leak from a hydraulic line when high pressure hydraulic fluid penetrated the skin on the employee's finger. The employee was hospitalized.
On May 6, 2020, an employee was operating a metal shear to cut 2-inch strips of aluminum. He was attempting to catch the strips behind the shear so they would not fall on the floor. While his hand was behind the machine, the shear was actuated via the foot petal, causing the shear to come down on his left middle finger. His fingertip was amputated to approximately the end of the nail bed.
An employee was operating a strip saw. As the employee shut off the saw, their right index and middle fingertips were amputated by the blade before it came to a complete stop.
An employee was using a table saw to cut a piece of wood for wall framing. The wood jumped, causing the employee's hand to move toward the saw, amputating their left index finger and causing a laceration on their hand.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.
An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.
An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
