On October 7, 2015, an employee's right thumb was amputated while operating a hardware insertion machine. On October 23, 2015, another employee's right index finger was amputated while operating a spot welding machine.

An employee was walking between a row of 7-foot tall metal cabinets, inspecting them for quality. The employee stepped between two painted steel cabinets and was struck in the head by another cabinet that was being carried by a forklift. The employee was hospitalized with a concussion.
An employee was loading a metal part into a machine when the metal part kicked backed, pinching his left middle finger between the part and the table. His left middle fingertip was amputated. The machine was unguarded at the time.
