A housekeeping employee was cleaning up a Formalin spill. The Formalin was covered with a spill-X absorbent to soak up the chemical. As the employee was scooping up the absorbent her head started hurting, she felt dizzy, and she passed out.
An employee was unloading a delivery from an elevated portable ramp when it collapsed, causing him to fall two stories to the pavement below. The employee was hospitalized with four fractured ribs.
