An employee was sweeping the kitchen floor when she stepped on the broom, causing her to trip and fall to the floor. The employee fractured her left hip and sustained a laceration.

An employee was cleaning a counter when they bumped into a customer, causing their feet to become entangled. The employee fell to the floor and suffered a left hip fracture.

An employee was lubricating a forming machine when hot bottles fell off the machine. A bottle contacted the employee's left leg and ignited their flame retardant pants. The employee's left leg was burned from the thigh to just below the knee.

An employee was returning from their break when the rags in his back pocket contacted a nearby stove heater. The rags caught on fire and spread to the employee's clothing. He sustained third-degree burns to the left and right buttocks.

An employee was pouring iron powder into a blender. The powder ignited and burned the employee's left hand, as well as a finger on his right hand.

On June 8, 2022, an employee was packing titanium powder into a 55-gallon drum contained in an aluminum dumper. The employee used their hands to grasp the trays and dump them into the drum. The powder dumping process creates a dust plume. The dust plume ignited, possibly due to static charge. A flash fire occurred and the employee suffered burns on the top of both hands.

An employee's hand was caught in a conveyor belt as they worked to clear a jam in a piece of equipment. The employee suffered a dislocation to the right elbow, as well as a broken ulna near the elbow, and was hospitalized.

An employee was changing AC filters while on a ladder. The employee stepped up to the second-to-last rung when they slipped and fell about 5 feet to the ground. The employee sustained fractures to their scapula and ribs, a collapsed lung, and a back injury.

An employee was performing restoration activities outside a 45-gallon condensation tank of a boiler system. As they were performing post-maintenance activities after repairing one of the two discharge pumps, one of the discharge valves would not open. The employee closed the intake pump to cool off the system and open the discharge valve. Two employees opened the inlet valve to return the tank to service. After they cracked open the inlet valve, a side wall of the tank blew out, causing the injured employee to suffer steam burns to half their body.

On August 19, 2025, an employee became ill and dehydrated after working outdoors for eight hours, participating in training activities. He was hospitalized.

An employee was ascending a fixed ladder and struck his head on an overhead object that was approximately 20 feet above ground level. The employee sustained neck injuries and tingling in his fingers. The employee was hospitalized.

The steel caster hit a crack in the concrete, the weld holding the caster onto the cart broke, and the coil and cart tipped over onto the employee. He was hospitalized with a laceration on his forehead and a pelvic fracture on his right side.

An employee was cutting a piece of metal rod with a metal cutting machine. His right middle finger became caught between the rod and the machine's table. The employee's fingertip was partially amputated.

An employee was working at her desk. She went to stand up and fell to the floor beside the desk. The employee sustained a hip displacement and required surgery.

An employee was processing wood boards at a chop saw when the saw malfunctioned and the blade cut her left hand and fingers. She was hospitalized and her little finger was surgically amputated.
