An employee was moving a dead animal using a removal cart. While transporting the animal, the cart tipped over and landed on the employee, resulting in a tibia fracture.

An employee was inside a livestock trailer that had just been cleaned. He was inspecting the trailer when someone called his name. He turned around, slipped, and fell out of the trailer. The employee landed on the concrete floor and sustained a broken wrist requiring surgery.

An employee was moving a 250-gallon tote of sodium hydroxide with a forklift. When the employee lifted the tote, the load shifted and fell forward into the building. The employee was attempting to pick up the lid that had fallen off of the tote when they were splashed with sodium hydroxide, resulting in chemical burns on their left scapula and the back of their left arm.

Two employees were repairing a slide gate at a feed mill. They were in a scissor lift extended to about 21 feet. A weigh lorry struck the lift and knocked it over. One of the employees fell to the ground along with the lift and suffered a broken right heel, as well as a concussion. The other employee did not sustain injury.

Two employees were performing maintenance on a jammed manual lift elevator door. The door dislodged and an employee's hand was pulled into the spring bindings connected to the door. Skin was amputated from the employee's left index finger.

An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool. The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.

Liveo Research, Inc.

An employee was rotating a carbide granulator blade when his finger was caught between the blade and a stationary part of the machine. The employee's right middle finger was partially amputated and they suffered an avulsion to the right ring fingertip.

An employee was using a CNC metal lathe machine in manual mode to check that the thread blocking tool made contact with the jaws. As the employee was manually spinning the spindle, his index fingertip was crushed between the spindle's jaws and the grooving insert, resulting in a fingertip amputation.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was using a de-clipping machine to cut the metal clips off of sausage sticks. As the employee put a link of sausages into the machine, their hand slipped into the point of operation. A piston pinched the employee's left index finger, causing a partial amputation.

An employee was operating a small tabletop grinder in a food safety quality assurance lab. She was stuffing meat down the grinder's chute when the grinder caught her right hand. Its auger caused an amputation to her middle finger and a partial amputation to her index finger.

An employee was cleaning a part on a pedestal grinder. The employee's middle finger contacted the abrasive wheel, resulting in a partial amputation.

A maintenance employee was removing an inspection plate from an industrial cooker. Hot liquid spilled onto him and burned his hands, arms, and torso area. He was hospitalized.

An employee was helping to move a 1,300-pound coil on a cart. The steel caster hit a crack in the concrete, the weld holding the caster onto the cart broke, and the coil and cart tipped over onto the employee. He was hospitalized with a laceration on his forehead and a pelvic fracture on his right side.
