An employee was pushing a cow femur into a bandsaw to cut out a beef shank piece. The saw hesitated and the employee stopped pushing; the saw started back up, and as the employee pushed the bone, the blade contacted the employee's right thumb and amputated the tip.

An employee was replacing a fan while on a 4-foot step ladder that was placed on an elevated platform. The employee fell approximately 8 feet and landed on a slanted plastic surface. The employee sustained two broken ribs in the upper back.

On September 23, 2019, an employee was cleaning machinery after production had stopped. The employee was struck by the bone cannon hopper as it was being emptied, causing possible ruptured spinal disks.

On June 21, 2019, an employee was visually inspecting product packaging seals on a conveyor. He went to grab a suspect package with both hands when his right pinky finger became stuck in the conveyor belt webbing, resulting in an amputation of tissue from the distal phalanx (nailbed to tip).

An employee was operating a stand-up forklift when the employee stopped and was struck from behind by a second forklift, suffering a leg injury that required hospitalization.

The injured employee was holding a board that was being cut by another employee. The board kicked back, causing the saw to amputate the employee's left little finger and lacerate their left hand, resulting in nerve damage.

On September 26, 2023, an employee was working in a trench box while a co-worker was lowering a piece of plastic pipe by hand using a rope. The pipe swung and struck the employee's hand, fracturing their right thumb and middle and ring fingers. The employee was hospitalized.

An employee went over to a co-worker's station while the co-worker was using a powered meat knife. As the injured employee was pointing, his right index fingertip was amputated.

Two employees were using a handheld banding tool to make banding tape for tent set-ups. One employee was holding the banding tool and the injured employee was looping a metal clip around the tape. The tool closed while the injured employee's hand was near the banding mechanism. The banding tape caught the employee's right ring finger as it tightened, resulting in a partial amputation.

An employee was holding a branch against a log on the ground to make a wedge while another employee was using a chainsaw to cut the wedge. The saw slipped and struck the employee holding the branch, resulting in a laceration to the right wrist.

An employee was scraping dried glue from the top lid of a hot glue pot. His left hand slipped and came into contact with the hot glue. He then suffered burns to his hands and fingers as he tried to remove the glue. He was hospitalized.

An employee was in a scissor lift approximately 20 feet above the ground to place a plastic tarp on a catwalk in order to catch condensation. They positioned the lift opposite the work area and stepped out of the scissor lift onto an I-beam. The employee fell from the I-beam to the ground. The employee was hospitalized with a skull fracture on the right side of their head and injuries to the chest and both eyes.

A maintenance employee was called to look at a pallet conveyor machine where a chain had fallen off a sprocket wheel. When the employee grabbed the loose chain, a light sensor caused the machine to restart. This created a pinch point between the chain and sprocket that caught the employee's left little finger. The finger was surgically amputated at the second knuckle.

The injured employee was standing by one of the stationary computers along the wall. There was a half-pallet located approximately 3 feet behind the employee that consisted of 12-pound boxes stacked six boxes high. A forklift was transporting a pallet that came in contact with the stationary pallet behind the injured employee, causing four boxes to fall and contact their legs. The injured employee was hospitalized with leg injuries.

An employee was using a box strapping machine to place straps on boxes when his right middle finger was caught in the machine, resulting in an open fracture to the distal phalanx.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.

An employee was unloading a truck for a delivery job. While moving boxes in the truck, the employee tripped over a pallet and fell out of the truck to the ground, resulting in four broken ribs.
