Three of an employee's left-hand fingertips were amputated while he was adjusting a press brake to bend 90-degree angles on bench seating.
On 07/14/2016, at approximately 11:15 a.m., an employee was using a saw to cut 4'x8' panels. As the employee was retracting the table back to its starting position, the blade cut the employee's right-hand middle, ring, and pinky fingers, amputating the pinky and ring fingertips.
