An employee was working in the palletizing area, inspecting a right angle gear box when his left index finger was amputated at the middle knuckle by the timing belt of the gear box.

The crane shuttle unexpectedly activated, pinning the employee between the rail frame and crane shuttle. The employee was hospitalized with a dislocated and fractured hip as well as fractures to their clavicle and sternum.
