An employee was assembling a mobile set of stairs while standing on a ladder. The employee was trying to get a strap off a platform and fell from the ladder to the lower level. The employee suffered a broken jaw and right shoulder fracture.

An employee was standing next to a vertical bagging machine and an incline conveyor. A pallet jack was entering the area and the employee stepped back out of the way. The rear of the pallet jack pushed an empty pallet toward the employee's feet and her left ankle was pushed into the bottom frame of the bagger machine. She sustained lacerations and an Achilles tendon injury that required surgery.
An employee was cleaning a moving conveyor belt. Her right hand was caught between the belt and an interior sprocket, causing lacerations and potentially broken bones. She was hospitalized.
