An employee was running an extrusion line in the manufacturing of a hydraulic hose. In the process of finishing a run, the employee attempted to separate the hose from a reel that was attached with string. The employee's left index finger was inadvertently tangled in the string, which tightened around the employee's fingertip. As a result, the skin and nail was removed from the fingertip.

The fingertip was amputated.
