Final narrative
An employee was working on a small platform with grating approximately one foot above a subfloor when a 2 inch by 2 inch section of grating came loose and fell. This caused the employee to trip and his leg contacted the now exposed grating which sliced his right calf.

An employee was standing in a concrete vestibule at a facility while reviewing renovations with a contractor. A catwalk that was previously adjacent to the vestibule had been removed. The concrete vestibule floor collapsed while the employee and contractor were standing on it, and they fell approximately 5 feet to the floor below. A concrete wall of the vestibule then fell on top of them. The employee suffered back and neck injuries and was hospitalized.
An employee was driving a pallet jack. It lost traction; when he tried to turn the handle to change direction, it would not turn and slid into a hallway rail. The employee's right leg was pinned between the rail and the jack, and his tibia and fibula were broken.
An employee was attempting to free a plastic bag that had become jammed in the bagging machine and suffered a partial amputation of the right index finger.
An employee was helping to install a new liner in a swimming pool. The employee entered the pool and stepped into a small uneven surface under the liner. The employee's right ankle twisted outward, resulting in a sprained ligament and a nondisplaced fracture to the right ankle.
An employee was looking for defects in a carpet while standing on the floor between a catwalk and the exit roller of a tufting machine. He stepped backward and tripped on the edge of the catwalk, grabbing onto the exit roller of the machine with his left hand in order to catch himself. While doing so, his left index finger went into the keyway hole on the rotating exit roller, which caused his left index finger to be lacerated.
An employee was unloading a forklift from a trailer to the ground. The employee was climbing down the ladder of the trailer when their foot slipped through one of the rungs, causing them to fall backward onto the forklift forks. The employee was hospitalized for fractured ribs.
