On or about December 30, 2015, the employee was working on the plastic extruder on the production floor. The employee's right index finger was amputated after being caught in the machine while flushing the extruder. The guard was not in place at the time.

An employee was changing a saw blade when the saw machine activated. The employee's left ring and little fingers were amputated.

An employee was throwing blankets onto a blanket folding and stacking machine. A blanket got stuck in the machine and she attempted to remove it when the machine contacted her right little finger, resulting in a fingertip amputation.

An employee was monitoring a rotary press when his left hand became caught in the machinery. The employee's left middle finger was amputated at the distal interphalangeal joint.

An employee was sweeping in an area with powdered PVC on the floor and began having difficulty breathing and coughing up blood, The employee was hospitalized for inhalation of PVC.

Lawter Inc.

An employee was working on a pastillator machine, which processes resin-based materials into solid pastilles. His right hand became caught between the feed roller (operating at approximately 350 F) and the cooling belt. The employee s glove and shirt sleeve adhered to his arm due to the heat. The employee sustained third-degree burns to his right hand and forearm, requiring hospitalization and surgery.

AdvanSix, Inc.

An employee was troubleshooting a leak on a low pressure (less than 50 PSI) condensate line in the oxidation area of the plant. He was working to remove insulation around a valve when either the valve or the line failed, and hot condensate contacted his right arm, right abdomen area, and the front of both legs, resulting in second- and third-degree burns.

An employee was replacing a fan on a cooling unit. As he went to adjust a pump, the unit caught his right hand and the fan blades severely lacerated his index finger. He underwent a medical amputation to the finger.

An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.

Two employees were setting up a mandrel in the spindle of a machine. The mandrel was side-shifted while an employee's hand was on the mast of a powered industrial truck. The employee suffered crushing injuries that resulted in amputations to the middle and ring fingertips.

An employee had been loading a double deck press. The employee saw a mold had been sent in with the C-hook still attached on the belly bar. As he went to remove the hook from the mold, his right thumb got caught on the C-hook. The employee's thumb was partially amputated.

An employee was operating a roller and paving a small pathway next to a pavilion. The ground was on a slight pitch, causing him to reach up toward the roll cage to stabilize himself. His right fifth finger was pinched between the roll cage and the rafter of the pavilion. The employee sustained a partial degloving injury with partial traumatic amputation.

Roby's Propane Gas

An employee was delivering home heating fuel when bees came from the ground and stung the employee's face, neck, torso, and hands. The employee proceeded to shut off the oil flow before losing consciousness. They were hospitalized due to an anaphylactic reaction.
