An employee sustained an avulsion fracture to the right small toe when he used a pry bar (7'7" long by 1" square tubing) to apply friction on a belt against a head drum located 8' above the floor. As the pry bar was being removed from the drum, the extended end of the pry bar landed on the employee's right toe.

At 8:00 a.m. on April 22, 2025, an employee was responding to a tracking fault on the bottom head of a six-head sander. While he was straightening the belt, his right hand was pulled in, wedged, and lacerated. He was hospitalized.

An employee was positioned between a dynamic tamp table and a static structure when his body hit a switch that engaged the tamp table. The employee sustained a fracture to his left pelvic area.

An employee was working to dislodge a piece of stray veneer from a conveyor. The employee set up a ladder against the machine and climbed the ladder. He placed his left hand on the rail of the conveyor and his fingers were caught in the chain and sprocket assembly that powers the conveyor. Three fingers were amputated. The machine was guarded at the time.

A maintenance employee was changing a shaft on a piece of equipment that had been isolated for repair. While positioning the shaft into place, the shaft fell approximately two feet and pinched the employee's left index finger. The employee suffered a partial amputation of the fingertip without bone loss.

An employee was using a cart to move saw blades. The cart tipped over, striking and pinning the employee under the blades. The employee sustained severe lacerations to their torso and legs that required surgery.
