An employee was greasing/cleaning a machine barring when his shirt was caught in a roller, pulling his arm into the machine. He suffered an open left distal radius fracture with soft tissue damage.

An employee was clearing a jam from the label cutting machine when their left middle finger was partially amputated.

Two employees were repairing the hydraulic engagement pins on a rented front-end wheel loader. The machine controls were activated to move the attachment pins and an employee's finger was caught between the pins and the bushings. Their right index finger was partially amputated.

On August 25, 2025, a machine operator was working on the heat-set end of a textile bleach/prep range machine. He was running leader cloth and it was bunching up. He climbed a catwalk so he could reach the cloth and flatten it out. His right hand was caught in a nip roll while adjusting the cloth. The employee's middle and ring fingertips were amputated.

Solmax Geosynthetics

An employee was making a fiber. He used his right hand to smooth the ends of polypropylene yarn and ensure they wrapped evenly onto a beam, spinning at 7 RPM. Some yarn got wrapped around his right middle finger and lacerated the finger and removed the nail, resulting in a distal amputation.

An employee was climbing down a ladder from the top of a machine 8 feet above the ground. He missed the second-to-last rung from the bottom and fell to the floor, suffering a broken right tibia and fibula.

On July 19, 2024, an employee was preparing to start the warp machine and was wrapping the yarned ends around the beam. Their right hand went behind the beam and between the press roll, bypassing the guard. When the machine started, it crushed their right hand. The employee was hospitalized.

An employee was using a utility knife to remove film from a roll of material for a coder machine when the knife became stuck. He pulled his arm back to free the knife. When the knife came free, it cut his left arm between the wrist and elbow.

An employee was changing the spacing on a telehandler's forks. A fork slipped, and the employee's left index finger was caught between it and the mast. The fingertip was medically amputated at the first knuckle.

An employee was pulling down a broken skid with a forklift. When the employee backed up the forklift to get the forks out of the skid he pulled down, he contacted the forks of another parked forklift, fracturing both of his legs. He was hospitalized.

An employee was carrying cups back to the kitchen when her foot got caught on a cart and she fell face-first. During the fall, a piece of glass from a cup cut the inside of her mouth, severing an artery. She also sustained a laceration on her lower lip. The employee was hospitalized.
