An employee was repairing an underground gas main when natural gas ignited and an explosion and fire occurred. The employee sustained burns.

An employee was lighting the pilot light on a water heater. Gas ignited, causing a flash fire and explosion at the residential property. The employee suffered burns to the head, neck, upper torso, and upper extremities.

An employee was preparing a reach in cooler (fry cooler) and charging the system with refrigerant (R290)with his gauges. When the employee removed the gauges from the liquid line service valve, the refrigerant immediately escaped, and the liquid refrigerant exploded due to an unknown ignition source. The employee was burned on the face, hands, abdomen and knees.

An employee was lighting a gas stove/pizza oven when an explosion occurred due to a natural gas buildup. The employee sustained first- and second-degree burns to the face, neck, arms, hands, and legs.

An employee was re-lighting a sulfur burner while another employee increased the pressure on the gas valve. A gas explosion occurred. The employee sustained second-and third-degree burns to their arm, hand, chest, abdomen, and neck.

An employee was installing a hydraulic extension cylinder in a tow truck when their leg was caught between the cylinder and the truck frame. The employee was hospitalized with a broken tibia, which required surgery.

An employee was underneath a flatbed truck, using a jack to lower the truck down. The truck began to roll and crushed the employee's right lower leg, resulting in hospitalization.

An employee was pushing a disabled vehicle while on a service call. The vehicle started to roll back and the employee placed his right foot on the back of the tire to hold the vehicle in place. The vehicle rolled over the employee's right foot, resulting in a fracture.

An employee was loading a car on the bed of a tow truck when his right hand got caught between the vehicle tire and the truck railing, resulting in broken fingers, an injury to his index fingernail, lacerations to his ring finger, and amputation of his middle fingertip.

An employee was unloading a client vehicle when the vehicle started rolling. The employee ran to the front of the vehicle trying to stop it but he fell down and the vehicle then rolled over his foot. The employee sustained a fractured toe, a left tibia fracture and puncture of the skin. The employee was hospitalized.

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.

An employee was helping two other employees to push a pallet of tile on a loading dock. He suffered a broken fibula and was hospitalized.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
