On February 12, 2016, at approximately 8:30 a.m., an employee and coworker were making a delivery with a freightliner step van. The employee got out of the van and walked to the back of the vehicle to open the van doors. The driver began backing towards the dock and the employee was pinned between the van and the dock, resulting in fractured ribs and a bruised torso.

On September 9, 2022, an employee was lifting a bag of linen when he lost his grip and fell backward, striking his back on a trash bin. The employee experienced pain in his back and subsequent numbness in his legs, resulting in hospitalization.
An employee was loading a box truck parked at a loading dock. A ratchet strap that he was using came unhooked, causing him to fall backward through a gap between the truck and the building. He landed on his back on the asphalt below and suffered a back injury, as well as a head laceration.
