An employee was manually adjusting a stack of empty boxes at the unloading dock. Ice was present on the floor due to a leaking water sprinkler line. The unloading dock is not heated. The employee slipped on the ice on the floor and the empty stacked cardboard boxes fell onto him as he fell onto the floor. The employee suffered a broken right hip as a result of the slip and fall.
