On 2/19/2016t, at 1:55 AM, an employee was working on a production line when she tripped on an airline, fell, and rolled her right foot/ankle. She was hospitalized and required surgery for a fractured right tibia and fibula.
While preparing a core mold for pouring, an employee used a crane and a lifting fixture to flip the mold. As the mold flipped, a 125-pound portion of the mold fell from the flask and landed on the employee's lower right leg, resulting in hospitalization for closed fractures to the tibia and fibula.
An employee was installing a safety emergency horn when he was shocked and fell from the second rung of a 4-foot fiberglass ladder. The employee sustained a fractured left arm.
