The employee was standing on a desk to measure the plastic cover on an overhead fluorescent light fixture. He stepped on the corner of the desk and the leg of the desk penetrated the 5/8" plywood flooring. The employee fell three feet and fractured the right leg.
An employee was cutting beef feet on a meat saw. The blade caught and a beef foot part jumped. The saw amputated the employee's left middle finger between the first and second joints.
