An employee was exiting a patient's room when her shoe was caught in the carpeting. She fell, striking her head twice on the floor. She suffered a concussion and was hospitalized.
On February 9, 2019, an employee received multiple concussions after tripping over a chair and falling at the nurse station. The employee struck her head on the coat/shoe rack.
