Federal OSHA recorded a severe workplace injury at Pepsico, 14501 N. Kelley Avenue, OKLAHOMA CITY, OKLAHOMA 73114 on — Fractures, affecting the chest, except internal location of diseases or disorders.

Employees were dropping off and loading product into a bay delivery trailer. One employee was operating a forklift and the second was operating a walkie-rider pallet jack. As they were transitioning from one side to the other, they rounded the corner, sounding their horns, and collided at the rear of the trailer. The second employee was caught between the mast of the forklift and load guard of the walkie-rider, resulting in fractured ribs.

An employee was stepping off a walkie rider pallet jack when their left foot was crushed between the pallet jack and the racks. The employee sustained a laceration and fracture to their left ankle.

An employee was performing maintenance on a lift gate. As the employee was removing the kingpin on the gate, it popped out, causing the gate to drop and strike the employee's right index finger. The employee suffered a partial amputation of the finger.

An employee was performing maintenance on a conveyor when the machine was reactivated. The conveyor chains amputated the employee's index and middle fingers.

Employee 1 was operating a lift in a standing position. Employee 2 was operating a cherry picker and backing up through an aisle. The lift collided with the cherry picker and employee 1 fell off the lift after striking his right ankle on a wooden pallet that was on the cherry picker. The employee sustained a broken ankle.

C & S Wholesale Grocers Inc.

An employee was operating a front-rider pallet jack from a freezer box to the loading dock when the vehicle collided with a stand-up forklift that was driving around a corner. The employee suffered multiple fractures in their left foot.

Hollingsworth Logistics LLC

An employee was operating a forklift in reverse down an aisle when the vehicle collided with another forklift. The forks of the other forklift went into the employee's right leg, resulting in a right hairline leg fracture.

New South Express, LLC

After moving empty containers with a forklift, an employee began to travel forward in the forklift. While re-entering the main aisle, the left fork struck the roll cage of an oncoming forklift that was travelling in reverse. This caused the injured employee's forklift to tip over onto a guardrail. The upper rail of the guardrail entered the top of the forklift's roll cage, pinning the employee's legs against the seat. The employee sustained a fractured leg.

An employee was operating a golf cart to retrieve and transport a ladder to the truck shop when they were struck by another vehicle at an intersection. The employee sustained head/brain injuries.

An employee was delivering beverages when he fell from the truck's liftgate to the ground. The employee sustained a fractured wrist, a dislocated shoulder, and bruised ribs.

The American Bottling Company

An employee was performing maintenance under a van. The van slipped off the jack and struck the employee, who suffered broken ribs in the left upper torso. The employee was hospitalized.

During scheduled preventative maintenance, an employee was searching for vertical bagger shafts. Upon locating a box thought to contain new shafts, the employee removed two high-powered magnets from bubble wrap. The magnets forcefully attracted to each other and amputated the employee's fingertip.

An employee was walking beside a pallet jack while operating it and became pinned between the pallet jack and some pallets. The employee sustained a fractured left leg.

CB Manufacturing Company, Inc.

An employee was replenishing product in a warehouse aisle. They collapsed and fell onto their back, striking their head on the concrete and suffering a laceration to the back of their head and injuries requiring hospitalization and brain surgery.

Rae Corporation

An employee was helping to move a 1,300-pound coil on a cart. The steel caster hit a crack in the concrete, the weld holding the caster onto the cart broke, and the coil and cart tipped over onto the employee. He was hospitalized with a laceration on his forehead and a pelvic fracture on his right side.

An employee was cutting a piece of metal rod with a metal cutting machine. His right middle finger became caught between the rod and the machine's table. The employee's fingertip was partially amputated.

An employee was working at her desk. She went to stand up and fell to the floor beside the desk. The employee sustained a hip displacement and required surgery.

An employee was processing wood boards at a chop saw when the saw malfunctioned and the blade cut her left hand and fingers. She was hospitalized and her little finger was surgically amputated.
