An employee jumped off a delivery truck's lift gate to avoid a collision with a product. He rolled his ankle on the ground, breaking it in three places.

At 7:10 a.m. on January 24, 2023, an employee had just cleared a jam on an empty pallet conveyor and was walking back to the palletizer when he noticed that one of the chains conveying the empty pallets was not aligned on its track. The machine was paused; he picked up the chain about 8 inches from the sprocket with his left hand. The conveyor then continued its cycle and his left-hand fingers were pulled into the chain and sprocket. He suffered three broken fingers and an amputation to the little fingertip.

An employee's right foot was run over by the left side of a forklift.

An employee was backing a loaded pallet jack out of a trailer onto a lift gate. When the employee applied the brakes, the pallet jack kept rolling. To keep from being run over, he jumped off the lift gate to the ground. He suffered a strained/sprained lower back, two broken left-side ribs, and a broken left wrist.

Cases of ice tea were knocked over and struck an employee on the back.

An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

Two employees were removing a service lift platform from the back of a radar system. While handling the service lift platform, employee 2 was standing on the back of the radar and lost their footing, causing the service lift platform to shift and fall. The injured employee then fell from the top of the platform electronic subsystem (PES) to the floor, approximately 56 inches. The injured employee fractured his left elbow and left hip.

An employee fell from a loading dock to the ground 2-3 feet below. She suffered a compound fracture of the left hip and was hospitalized, requiring surgery.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was performing maintenance under a van. The van slipped off the jack and struck the employee, who suffered broken ribs in the left upper torso. The employee was hospitalized.

During scheduled preventative maintenance, an employee was searching for vertical bagger shafts. Upon locating a box thought to contain new shafts, the employee removed two high-powered magnets from bubble wrap. The magnets forcefully attracted to each other and amputated the employee's fingertip.

An employee was walking beside a pallet jack while operating it and became pinned between the pallet jack and some pallets. The employee sustained a fractured left leg.

An employee was replenishing product in a warehouse aisle. They collapsed and fell onto their back, striking their head on the concrete and suffering a laceration to the back of their head and injuries requiring hospitalization and brain surgery.

An employee was working to extend a dock plate from inside a semi-trailer. The dock plate was stuck. The employee's right hand was underneath the dock plate when the plate suddenly released and fell onto their hand. The employee sustained partial amputations to the index and middle fingers. They also sustained lacerations to the index and little fingers.

An employee was cleaning leaves with a backpack blower when he fell from a ladder, resulting in injuries to his head, neck, and back.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

An employee slipped while descending in a stairwell, fell, and suffered multiple contusions/bruises to the head, neck, shoulders, knees, and back. She was hospitalized.

An employee's right middle finger was caught in a nip point. The employee suffered an amputation to the middle finger at the first knuckle.

An employee was directing trucks as they entered and left a highway work zone. A vehicle struck them, resulting in multiple fractures and other injuries. The employee was hospitalized.
