An employee was using a mill cutter to cut 6-inch slots. His glove was pulled into the cutter and his left thumb was partially amputated.

An employee was attempting to unclog a relief line that was piled with sediment on a boiler. When he unclogged it, there was a pressure release, and hot water shot out of the line and contacted the employee's left shoulder and arm, resulting in burns.

An employee was using a 4-inch angle router to remove residual weld from a tooling-removal area on an aluminum ring. The angle router kicked and its blade came into contact with a finger on the employee's left hand, causing an open fracture.

An employee was investigating a thermostat that was mounted about 10 feet above grade. She was using a step ladder to access the thermostat. She was standing approximately 4 to 5 feet up the ladder when it tipped over. The resulting injuries required hospitalization.

An employee was pulling on a bale of corrugated cardboard boxes that were on a conveyor. The straps on the bale failed and the employee fell to the floor. The employee was hospitalized and required surgery for a fractured femur and/or hip.

On December 27, 2023, an employee was delivering mail to an apartment complex when their knee gave out and they fell to the ground. The employee was hospitalized with a left tibia fracture.

An employee was walking to the inventory cage when her ankle gave out and she fell to the floor. The employee sustained a femur fracture and was hospitalized.

An employee was dislodging a pallet that was stuck when boxes of product fell onto them, causing them to fall to the ground. The employee suffered a fractured femur.

An employee was walking on a sidewalk and stubbed his toe on an elevated portion of concrete, causing him to trip and fall. The employee's right knee was dislocated.

An employee was removing a hose from a chemical tank after filling the tank with hexamethylenediamine (HMD). He checked the line to ensure that the hose was clear, then disconnected the hose at the connection. Residual chemical sprayed onto his face, causing a second-degree burn. He was hospitalized.

Two employees were moving a railcar (plastic pellet hopper car) using a mobile railcar mover. One employee was the engineer in control of the railcar mover and the injured employee was a switchman. The railcar mover went forward and the hopper railcar struck the injured employee, resulting in a severe crushing injury to the employee's right arm.

An employee was opening a filter housing in the potassium chloride production area and got sprayed with a hot solution of potassium chloride brine. The employee sustained thermal burns to their right hand and legs.

An employee was performing maintenance on a guillotine cutter machine. As the employee was placing a pin in the machine, his left middle fingertip was caught, resulting in a partial amputation to the finger. The machine was not locked out at the time.

An employee was stacking concrete blocks for a concrete bunker. As a block was swinging into place, the employee's left leg was pinched between two blocks, resulting in injuries to his left heel and lower leg.

An employee was climbing a ladder to inspect a vent in a restaurant dining area. The ladder slid out on the floor and the employee fell approximately 12 feet onto the ladder and the floor. He was hospitalized with a dislocated shoulder and a back injury.

An employee was rappelling down a tree. When he was most of the way down, he fell on the ground and suffered a broken pelvis.

An employee was filtering a fryer with a fryer filter machine. After going around the corner and then returning to the fryers, the employee stepped into the filter machine. The hot oil burned the employee's right ankle, and the employee was hospitalized.

An employee was cleaning out corn from a grain bin. The sweep auger in the bin was activated and caught his trouser leg. His leg was pulled into the auger, which injured his calf and caused leg fractures and lacerations.
