An employee heard the cap fall off of a chemical system flow line to a holding tank containing ammonia. The employee went to the area to close the valve and was overcome by ammonia fumes, suffering a respiratory injury.
An employee was passing through a building when she suffered an allergic reaction to a cinnamon air freshener. The employee began to feel dizzy and ill, ultimately becoming unresponsive. The employee was hospitalized.
