On 03/31/2016, at approximately 4:00 pm, an employee tripped on a tarp and fell while loading/unloading material. The employee suffered a fracture of the right femur.
An employee arrived at work and was walking into the store through the parking lot. The employee tripped over a curb and fell to the ground, sustaining a femur fracture.
An employee was removing boxes from a pallet. When he stepped backward, his foot caught on shrink wrap. He fell to the concrete, landing on his right hip and breaking it. He was hospitalized, requiring surgery.
An employee was unpacking donations from a donation bin and moving a portable canopy when he lost his balance and fell forward. The employee struck his head on the concrete and sustained a fractured C4 vertebra and a laceration to the forehead that required stitches.
