A forklift operator on the other side of the storage rack was pushing a skid with a cooler but pushed it too far, causing a pallet (weighing approximately 100 pounds) on the other side of the rack to fall on the employee cleaning her cart. She was hospitalized with injuries to her mouth, chest, legs, and feet.
