An employee was cleaning a food cart in the back storage part of the store. A forklift operator on the other side of the storage rack was pushing a skid with a cooler but pushed it too far, causing a pallet (weighing approximately 100 pounds) on the other side of the rack to fall on the employee cleaning her cart. She was hospitalized with injuries to her mouth, chest, legs, and feet.

An employee slipped and fell outside, landing on her hip on concrete. The hip was broken and she was hospitalized, requiring surgery.

An employee was preparing to change the pallets on a screen printing machine. She stepped between the arms of the machine when the machine re-energized, turned, and struck her, pushing her into a mobile cart and causing her to fall to the ground. The employee suffered suffered five fractured ribs, difficulty breathing, and internal bleeding.

An employee used a cart with a ladder attached to pull items from a shelf. The employee stepped back and tripped over either the track or the knuckle on the end of the aisle, fell back and landed on her hip, fracturing her femur.

The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.
