An employee was struck by a front-end loader while he was removing a chain from a dumpster. He was crushed from the waist down.

Metromont

An employee was applying trowel finish to double tees. As the employee started the trowel, he discovered it was set to full throttle. He struggled to get the trowel machine under control. The trowel blade contacted a portion of the double tee side rail. The blade stopped, but the trowel continued to rotate, throwing the employee off of the form and onto the ground on his right side. The employee was then struck by the trowel machine control bar.

Metromont

An employee was attempting to light a handheld propane torch when an explosion occurred. The employee received second degree burns to the face, hand, and other body parts. A second employee also suffered injuries.

An employee was walking back to her workstation. A forklift reversed a short distance, then completed a U-turn and struck her, running over her lower extremities. She was hospitalized with a broken ankle.
