An employee was in a lift about 4 feet in the air, cleaning the vents on a ceiling. He fell off the lift to the floor, suffering spinal fractures and a broken humerus.

The employee broke through the partially hardened metal; still-molten metal flowed to the employee's steel-toed right boot and entered through the cloth boot tongue. The employee suffered a third-degree burn to the right foot and was hospitalized.

An employee pulled out a bucket of hot oil from under the fryer. The employee then stood on the table to clean the back wall. He stepped down into the bucket of hot oil, resulting in third-degree burns to his left leg.
