On November 9, 2020, at around 12:00PM, an employee was helping to rescue ducklings from a sewer when the sewer grate came down on their hand. Two fingers were fractured and a fingertip was amputated.

An employee broke the upper right arm while raking leaves.

An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

An employee was descending a 4-step maintenance stand when she missed the bottom step and fell to the hangar floor. The employee suffered a left hip fracture.

An employee stopped a belt and was going to check it for missing packages. She fell backward 3-4 feet from an elevated platform and sustained a broken right arm.

An employee had just finished a routine concrete pour and was ascending the ladder to clean the concrete mixer truck. The employee lost their grip and fell approximately 2-3 feet, contacting the truck's bumper. The employee suffered rib fractures and a punctured lung.

An employee was performing duties as an expeditor. After opening the dock door, the employee scanned the barcode on the door of the truck and placed one foot on the truck and one foot on the dock. The truck drove out of the stall, causing the employee to fall 4 feet off the dock onto the concrete. The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

An employee was fixing hydraulic tongs when the tong gears amputated their left hand, requiring surgery.

While taking pictures in the attic of a residential home, an employee fell through the ceiling and landed on the floor of the laundry room. The employee was hospitalized for a visible head trauma and a back injury.

An employee was on a 6-foot A-frame ladder running data cables through a drop ceiling. The employee was descending the ladder to move to the next section of the ceiling when the ladder wobbled and the employee fell to the floor, sustaining spine/tailbone fractures.

An employee was driving a a vehicle when they collided with a customer's garage. They were hospitalized with a back injury.

An employee parked their car and was walking into a restaurant when their right hand got stuck in an elevator door, resulting in partial amputations of four fingers.

An employee was cleaning leaves with a backpack blower when he fell from a ladder, resulting in injuries to his head, neck, and back.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

An employee slipped while descending in a stairwell, fell, and suffered multiple contusions/bruises to the head, neck, shoulders, knees, and back. She was hospitalized.

An employee's right middle finger was caught in a nip point. The employee suffered an amputation to the middle finger at the first knuckle.

An employee was directing trucks as they entered and left a highway work zone. A vehicle struck them, resulting in multiple fractures and other injuries. The employee was hospitalized.
