On or about May 4, 2016, an employee cleaning the roll feed on a power press had the tip of his right pinky finger pinched between rolls resulting in amputation of the fingertip.

An employee was changing a saw blade when the saw machine activated. The employee's left ring and little fingers were amputated.

An employee was clearing a jam from the label cutting machine when their left middle finger was partially amputated.

Two employees were repairing the hydraulic engagement pins on a rented front-end wheel loader. The machine controls were activated to move the attachment pins and an employee's finger was caught between the pins and the bushings. Their right index finger was partially amputated.

An employee was cleaning the conveyor on a piece of equipment when they slipped and their right hand was pulled into the chain sprocket. The employee's fifth fingertip was amputated.

An employee was throwing blankets onto a blanket folding and stacking machine. A blanket got stuck in the machine and she attempted to remove it when the machine contacted her right little finger, resulting in a fingertip amputation.

An employee was grinding the ends of a metal spring. His hand slipped, and his right index finger contacted the grinding wheel and was lacerated. Part of his fingertip was amputated.

An employee was adjusting a machine in the facility when the flat stock material running through the machine cut his left index fingertip. The employee sustained an amputation to the fingertip.

An employee was placing a bushing into an opening (an eye) at the end of a piece of metal. The employee was holding the bushing with their right hand and engaging the hydraulic press when their glove was pinched between the bottom of the bushing and the metal. The glove was drawn into the eye, and the employee's right index fingertip was pinched and amputated at the first knuckle.

An employee was inspecting leaf springs. They were trying to straighten a spring plate on a vise when their finger was pinched between the vise and the workbench, resulting in an amputation.

An employee was correcting a part using a bending machine when their right index finger was crushed and fractured in the machine.

An employee was re-arranging boxes of pipe insulation on a pallet. One of the boxes fell from the pallet and knocked over an upright empty cylinder. The cylinder fell and crushed the tip of the employee's right toe. The employee's toe required surgical amputation.

An employee was securing the claw of a grapple truck to the truck bed. His left little finger was caught between the tie down strap and the rub rail of the truck, resulting in partial amputation of the finger.

Parker-Hannifin Corporation

An employee was cutting extruded aluminum framing material using a horizontal band saw. She went to grab a rag in the machine when the rag contacted the blade and pulled her hand in toward the blade, resulting in a partial amputation of the right index finger.
