An employee was cleaning the loading dock floor and slipped. The employee then tripped on a ramp stand and fell from the loading dock to the parking lot surface approximately 5 feet below. The employee sustained contusions to his neck, legs, and back.

An employee was lifting a motor and reaching to place it on a machine when they sustained a back injury. The employee was hospitalized.

On 12/22/2023, an employee was carrying bins filled with pasta to a refrigerator. As he lifted a 20-pound bin, he felt pain in his lower back. The employee sustained a herniated disc.

An employee was lifting a patient when they pulled a back muscle. She suffered back pain and spasms radiating into her leg with weakness and numbness. The employee's L3-L4 disc was herniated and she was hospitalized.

There was a sudden burst of fire in the kitchen, and both employees were engulfed by deflagration of gas which oriented from a propane-powered griddle. They suffered first-, second- and third-degree burns to their head, face, arms, and legs.

An employee loaded his truck. The dock plate was stuck under the carrier dolly and he could not lift the dock plate from the side while standing on the dock. The employee entered the truck and pushed the dolly away from the dock plate. This allowed him to lift the plate from inside the truck. While lifting the plate, his body shifted forward, and he fell between the dock and the truck, resulting in a fractured left wrist.

The injured employee had been conducting food service activities. He was stabbed twice in the chest and three times in the back by another employee using a pocket knife. The employee was hospitalized.

On April 2, 2025, an employee was reassembling a food-sealing machine when the machine activated and a metal plate pinched their right thumb tip, amputating it. The machine was not locked out/tagged out at the time.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.
