An employee was struck by forklift that was backing up resulting in a lower leg crush injury that required amputation midway between the knee and the ankle.

Two employees were removing a motor from an overhead bracket and turned to lower it to the floor. As they were lowering it, one of the employees had his left middle finger caught and crushed between the motor and a 12-inch chute. The employee sustained amputation of the distal phalanx.

An employee was changing mud cups in the hopper cylinders of a concrete pump truck. The cylinders moved and three of the employees fingers were amputated.
