An employee was standing on a truck platform and was in the process of turning around to step down when he fell onto the gravel/rock. The employee sustained a broken right wrist and facial lacerations.

An employee was using a crane. The hook slipped and a fitting fell onto a finger on the employee's left hand causing a fracture and laceration.

An employee was transporting an 800-pound die on a cart from the tool room. The employee was pulling the cart when it struck a metal plate on the floor. An employee slipped and fell while exiting the cab of his tender truck. The employee was hospitalized with a fractured femur.

An employee was descending the side of a barge, having ascended to the top of it to close an opening on top of the barge lid. There was a towing rope on top of the three-step ladder on the barge; the employee put his weight on this rope, believing it to be part of the ladder, and fell backward. His ankle and shin were caught between the top of the third step and the barge lid. He suffered a fracture near the right ankle and another near his right shin and was hospitalized, undergoing surgery.

An employee was climbing onto the back of a fertilizer truck and fell to the ground. The employee was hospitalized for broken ribs and a punctured lung.

An employee fell through a hole in a work platform to the ground about 21 feet below. The employee was hospitalized with multiple fractures and a traumatic brain injury.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.
