On or about 6/6/16, at approximately 10:30 AM, an employee was using the stairway between the first and second floors when she tripped and struck her head on the hand rail and then on the top step. She suffered a concussion and was hospitalized.

An employee was working at a policyholder's residence responding to their insurance claim. He was on their outside deck and began to climb up a ladder. He was on the 2nd rung from the ground when the ladder lifted back towards him, hitting his forehead area. He continued to fall backwards, hitting the back of his head on the deck and knocking him unconscious.
A nurse was visiting the home of a patient for requested treatment. She tripped on her gown while going up the steps to the property, falling and fracturing her left femur.
