An employee was walking to a desk while holding a box. The employee fell, hit their head on the floor, and suffered a concussion.

An employee got dizzy while walking and fell, striking her head against the wall and suffering two fractures to her left foot.

An employee was using a circular saw to cut flashing. He was finished with the cut and started walking away when his fall protection ropes became entangled in the saw and the saw was pulled into his leg. The employee sustained a left hamstring laceration.

An employee had finished sharpening a knife on the grinding wheel and was returning it when he lost grip of the handle. As the employee reached to catch the it, his abdomen was lacerated by the knife blade in the right lower quadrant.

An employee was removing an ambulance off a lift jack when the pressure against the jack caused the handle to kick upward, striking the employee on the left side of the head, neck, and jaw. The employee suffered injuries that required hospitalization.

An employee was installing a flange on an evaporator skid. The employee was holding the bar of a torque wrench when his hand was caught in the pinch point as another employee turned the wrench. He suffered a right middle fingertip amputation.

An employee was using a hand grinder to eliminate discrepancies on columns around the project when the grinder jumped and came out of the employee's hands. They sustained a little finger amputation.

During a workshop meeting in a hotel, an employee heard a drilling noise, so he walked outside to see what it was. An explosion occurred (possible gas line) and his face, ear, and hair were burned. He also fell and sustained a pelvic fracture.

On October 20, 2025, an employee was installing a security system. He was on a 6-foot A-frame ladder to drill holes in a wall when he fell from the ladder and struck his head on the floor. The employee was hospitalized with a fractured skull.

An employee was playing kickball as part of a team building activity when they fell and suffered a dislocated left foot.

An employee was replacing a light fixture in an office ceiling and sustained a possible electric shock. The employee fell from the stepladder and suffered fractures to the face and left shoulder.

An employee was on a knee cart. Its handle grip came loose as she turned and she fell to the tiled floor, breaking her lower right tibia and fibula.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
