An employee was loading a vibratory roller onto a trailer. The employee missed a step exiting the machine and fell onto the trailer, striking his back and head. He then fell from the trailer onto the ground. He suffered a head abrasion and fracture to the mid- to lower back.
An employee was tying down a tractor on a flatbed trailer with tie down chains. He then stepped back off the trailer to the ground, and felt pain in his left hip. The employee sustained a torn hip flexor.
An employee was unloading tools. As they stepped off a lift gate of a box truck to the trailer, their left knee buckled, causing the right knee to buckle as well. The employee sustained a right knee fracture and a ruptured quadriceps tendon on the left and right leg.
