An employee was checking piece numbers on structural steel beams that were stacked one on top of another when a piece of cribbing broke and collapsed the stack of beams. The employee could not get out of the way fast enough and the collapsing steel fell on his leg, pinning it under the beams. He was hospitalized with a broken/fractured right leg.

EBENEZER RAILCAR SERVICES INC.

Employees were removing fasteners from an aluminum side sheet of a railcar coal gondola. The side of the rail car collapsed onto the employees' lower extremities, resulting in fractured legs and pelvis and a head injury. Two employees were hospitalized.

MM Block Work Corporation

An employee was walking by a masonry block wall at the side of a garage. A gust of wind caused part of the wall to collapse onto three employees, one of whom suffered a broken right ankle and a head injury that required staples. That employee was hospitalized.

Recycling Technology Consultants, LLC

An employee was tasked with cutting a bale that had a combination of plastic and cardboard. He was using a pair of shears to cut the wire banding when the bale collapsed onto his left leg, breaking his femur. He was hospitalized.

An employee was cleaning her workstation and organizing her files when a section of the acoustical ceiling (tiles and grid), adjacent light fixtures, and portions of the plaster ceiling above suddenly collapsed due to a prolonged, concealed steam leak that weakened the fastener connections. She sustained a cut to her head, bruise to her head, scratches on her neck, a scratch to her left hand, and irritation to her right hand, requiring hospitalization.

An employee was working in a trench box at an excavation site. When he exited the trench box, he sunk into mud up to his knee, suffering possible muscle trauma requiring hospitalization.

Steel-King Industries, Inc.

On October 21, 2025, an employee was operating a saw when a pneumatic clamp caught their right middle finger. The employee suffered a partial amputation to the finger.

ORRVILON, INC

An employee completed an end cut on a metal shim and started transferring the metal shim from the saw conveyor to two saw horses in the area. The employee set one end of the shim (7 feet long x 8 inches wide, weighing 218 pounds) on one of the saw horses, then slid the other end onto the other saw horse and the shim fell from the first saw horse. The employee's left index fingertip was pinched between the saw horse and the shim, resulting in a skin avulsion. The fingertip was amputated later.

Cives Steel Company New England

An employee was using a crane to move a 44-foot, 3,343-pound I-beam. As the beam was moving west, it straightened out (north to south), beginning to swing south toward the northwest corner of a building. The crane then sent the beam southward, directly toward the corner of the building. The beam caught the tips of the employee's left index, middle, and ring fingers against the building. The last joints of the middle and ring fingers were amputated, and the last joint of the index finger was partially crushed.

Suburban Steel Supply

An employee was moving steel beams with an overhead crane when the bundle struck a bollard and became unstable. As the employee reached to stop the material from shifting, his thumb was pinched in-between the beams, resulting in a partial amputation.

Canam Steel Corporation

An employee was rigging a steel joist to be lifted by a crane. The joist fell and pinned the employee against the concrete ground. The employee sustained fractures to multiple ribs, his right arm, right tibia, and an open fracture to his right ankle, as well as a dislocated right hip.

An employee was checking on a machine outside of the building and removed a cover to clean out a blocked area. His hand was caught by a rotary valve in a dust collection machine. The employee's fingertip was amputated.

An employee was working to clean a glue roller with a scraper tool. The tool and the employee's left thumb were pinched between two rollers, resulting in a partial amputation.

United Refrigeration Inc

An employee was re-arranging boxes of pipe insulation on a pallet. One of the boxes fell from the pallet and knocked over an upright empty cylinder. The cylinder fell and crushed the tip of the employee's right toe. The employee's toe required surgical amputation.

An employee was securing the claw of a grapple truck to the truck bed. His left little finger was caught between the tie down strap and the rub rail of the truck, resulting in partial amputation of the finger.

Parker-Hannifin Corporation

An employee was cutting extruded aluminum framing material using a horizontal band saw. She went to grab a rag in the machine when the rag contacted the blade and pulled her hand in toward the blade, resulting in a partial amputation of the right index finger.
