Final narrative
On 6/16/16, an employee lost his footing inside of a cardboard trailer and fell to the concrete ground. He broke his left tibia and was hospitalized.
At around 7:45 AM, an employee was troubleshooting a blockage in the pipeline that circulates oil from the fryer to the oil filter machine. Pressurized air in the system caused the oil within the pipeline to surge back into the enclosed fryer. Oil then sprayed through small openings in the fryer and burned the employee. The employee was hospitalized with burns to the back of the neck and upper shoulders.
An employee was running the breading machine. He looked into the product entry opening and saw breading had clotted up inside. He was attempting to remove the clot when his right hand contacted a conveyor belt flap inside the machine, resulting in a right middle fingertip amputation.
