Final narrative
An employee was operating a stand-up forklift (order picker) to take a 40-pound roll to the wrapping machine. She loaded the roll on top of the front edge of the order picker, above the operator department and behind the mast of the order picker. As she was going forward, she came to a stop and the roll fell, striking her hand and the control panel. This caused the order picker to go in the reverse mode. The employee quickly exited the order picker, and the back wheels ran over her foot, resulting in an amputation.
